Points of Consideration:-

1. Mention your education year, Institution name, and session year.
2. Mention your Roll number issued from the University in Bold.
3. Mention the lost degree, if you have Registration number, mention in Bold.
4. FIR shall be filed before issuing this Affidavit.
5. Affidavits may be stamped and notarized. 

SAMPLE:-

AFFIDAVIT 

I, {Mention name, father/husband name and full address} do hereby solemnly affirm and declare as under:- 

 1.  That the deponent is permanent residing at the above said address.

2. That deponent was the student of {Mention Institution Name and City}, from session {Mention Session Month and year} to {Mention Session Month and year}.

3.  That deponent was enrolled vide enrolment no.{Mention your Roll Number in University} in {Mention University and City}.

4. That deponent was issued {Mention Type of Degree}  Degree on {Date of Issuance} for session {Mention Session} with Roll no. {Mention Roll Number} {If multiple degrees are lost, repeat the 4th paragraph}.

5. That deponent has lodged the police report with regard to said Degrees vide Unique ID No. {Mention Number} dated {Mention date of report}.

     6.  That I will not misuse the said degrees.

     7. That the said degrees have been lost and Duplicate Degree may be issued.

     8. That please issue the Duplicate degrees in favour of the deponent.
